A Suicide Care Management Program at MHRC
Mental Health Resource Center (MHRC) operates the Link to Life Program as its designated suicide prevention care management pathway, supporting individuals identified as moderate to high risk for suicide. The program aligns with the Zero Suicide framework and enhances safety, continuity of care, and recovery planning for individuals across MHRC services.
Program Overview
Link to Life provides a proactive, structured approach to managing suicide risk by offering coordinated follow-up and safety-focused interventions. This program supplements existing MHRC services and adds a dedicated layer of care to help prevent crisis episodes and reduce the need for psychiatric hospitalization.
Link to Life is not a case management program, but rather MHRC’s system wide safety net for suicide prevention, operating across all programs.
Who is eligible?
- Adults (age 18+)
- Residing in Duval County
- Identified as moderate to high risk for suicide and/or recently discharged from a psychiatric setting
