Care Coordination Services

Mental Health Resource Center (MHRC) offers targeted care coordination programs to help individuals transition successfully from crisis settings into long-term community-based support. Our care coordination services focus on ensuring individuals have timely access to treatment, housing, medical care, benefits, and other essential resources to maintain stability and recovery.

All services operate under MHRC’s No Wrong Door approach, promoting warm handoffs and seamless transitions between levels of care.

Mental Health Resource Center

Programs Offered

Care Transitions

Designed for individuals at high risk following a mental health crisis or Crisis Stabilization Unit (CSU) admission.

Services Include:

  • Linking to outpatient mental health and substance use services
  • Connecting individuals to housing, medication management,
    and social services
  • Preventing readmissions and promoting long-term stability

Eligibility:

  • Two or fewer CSU admissions
  • Diagnosed mental health condition
  • Willingness to engage in outpatient services

Behavioral Health Care Management (Care Coordination-Outreach)

Focused on individuals with frequent hospitalizations or coming from state hospitals or incarceration.

Services Include:

  • Intensive community outreach and engagement
  • Linking individuals to treatment, housing, and benefits
  • Reducing unnecessary hospitalizations and promoting recovery

Eligibility:

  • Three or more CSU admissions in 6 months,
  • CSU admission lasting 16+ days,
  • Recent state hospital admission,
  • Recent incarceration
  • Diagnosed mental health condition and willingness to engage in services
