Service Coordination Services

Located in our Comprehensive Services Centers

At Mental Health Resource Center (MHRC), Service Coordination Services are available at our North and South Comprehensive Services Centers (CSCs) to support individuals in accessing essential community resources and overcoming barriers to care.

Our Community Resource Specialists help link individuals to services that promote stability, wellness, and recovery, whether they are new to MHRC or currently receiving services.

Services Provided:

  • Information, education, and referral to community resources
  • Navigation support for housing, food assistance, employment, healthcare, and benefits
  • Assistance completing applications for services such as Medicaid, SNAP (Food Stamps), SSI/SSDI, and housing programs
  • Support coordinating care between providers and external agencies

Service Coordination staff are available to provide personalized assistance, offering warm handoffs and helping individuals connect to both MHRC internal services and community-based programs as needed.

Who Can Access Services?

  • Walk-in individuals seeking mental health services
  • Individuals completing intake at MHRC CSC locations
  • Current MHRC clients experiencing new or urgent resource needs
  • Community members referred for assistance linking to services

Service Coordination is available regardless of insurance status and promotes a No Wrong Door approach to accessing support.
